HOUSING SUPPORTIVE SPECIALIST

Program Purpose: To assist participants in stabilizing housing situation after a move to transitional, permanent or permanent supportive housing in order to prevent a return to homelessness.
Program Description: Provides the following primary services to qualified homeless persons, as resources allow:
- Provide case coordination
- Develop a housing maintenance plan with participants
- Conduct home visits to check on well-being and successful transition towards housing maintenance, and increase self-sufficiency
- Provide food, clothing, hygiene items on a need basis
- Limited assistance with obtaining furniture on a need basis
Eligibility & Program Requirements:
- Must be homeless per HUD’s definition
- Must have obtained transitional, permanent or permanent supportive housing
- Must agree to actively participate in services geared to developing self-sufficiency skills housing maintenance skills, and addressing issues that put the participant at risk of a return to homelessness
- Must agree to home visits to monitor progress
- Services are limited to a maximum of 6 months
Cost: This is a free service to participants
