HOUSING SUPPORTIVE SPECIALIST

Program Purpose: To assist participants in stabilizing housing situation after a move to transitional, permanent or permanent supportive housing in order to prevent a return to homelessness.

Program Description: Provides the following primary services to qualified homeless persons, as resources allow:

  • Provide case coordination
  • Develop a housing maintenance plan with participants
  • Conduct home visits to check on well-being and successful transition towards housing maintenance, and increase self-sufficiency
  • Provide food, clothing, hygiene items on a need basis
  • Limited assistance with obtaining furniture on a need basis

Eligibility & Program Requirements:

  • Must be homeless per HUD’s definition
  • Must have obtained transitional, permanent or permanent supportive housing
  • Must agree to actively participate in services geared to developing self-sufficiency skills housing maintenance skills, and addressing issues that put the participant at risk of a return to homelessness
  • Must agree to home visits to monitor progress
  • Services are limited to a maximum of 6 months
